Understanding the Risks in Transgender Medical Care for Children

A recent judgment from the Federal Circuit and Family Court of Australia has brought further attention to the complexities surrounding medical treatment for transgender children—particularly when parents disagree on the treatment and given that the long-term effects of treatment remain uncertain.

The case concerned “Devin,” a 12-year-old child who had been expressing a female gender identity from a young age. Devin’s mother supported treatment at a hospital gender clinic, including the use of puberty blockers to delay male puberty. Devin’s father, while supportive of Devin’s right to explore their identity, raised concerns about the potential long-term consequences of medical intervention at such an early stage.

The court had to grapple with several difficult and sensitive questions:

  • Did the clinic provide both parents with balanced, comprehensive information to support informed decision-making?
  • Were medical professionals forming conclusions based on selective or biased information?
  • How safe and reversible are puberty blockers for children?
  • Could delaying treatment reduce potential harm—or increase emotional distress?

Expert evidence was divided. While some advocated for early gender-affirming care, others highlighted the limited research on long-term impacts such as fertility, bone density, brain development, and other aspects of physical and psychological health.

Ultimately, the court decided to restrain Devin from receiving puberty blockers at this stage, allowing more time for development, exploration, and understanding. The court made clear that this was not a judgment against transgender rights, but a cautious approach to a complex and evolving area of medical care.

Notably, concerns were raised about the clinic’s apparent ‘one-size-fits-all’ model, where a diagnosis of gender dysphoria appeared to lead directly to puberty suppression, without thorough consideration of alternative options. The court referenced several expert reviews—such as the 2024 Cass Report and findings from NHS England—which also advise caution in early medical intervention.

This case serves as a powerful reminder: when it comes to children, especially in areas of emerging medical science, treatment decisions must be underpinned by transparent advice, balanced information, and careful consideration of the child’s long-term wellbeing.

This case highlights why extreme care is needed when offering medical treatments to children—particularly those with long-term or irreversible impacts, or where the risks are not properly known.
